About the Role

We are seeking an experienced Case Manager to join our Community Support for Families(CSF) Program. As a key member of our team, you will provide critical support to families in need by connecting them with essential resources and services.

Your Key Responsibilities:

  1. Assess client needs and develop comprehensive case management plans.
  2. Coordinate care and services with internal and external stakeholders to ensure seamless delivery.
  3. Provide crisis support and problem-solving guidance to clients and families.
  4. Maintain accurate records and documentation in accordance with regulatory requirements.
  5. Participate in regular supervision and training to enhance your skills and knowledge.

What You'll Need:

  • Bachelor's degree in a relevant field, such as Social Work or Counseling.
  • Valid driver's license and ability to safely operate a vehicle.
