Lead and Inspire the Enhanced Care Team:

As the Dementia Clinical Nurse Specialist, you will be responsible for leading and managing our Enhanced Care Team, a dynamic group of Band 3 Healthcare Assistants dedicated to providing personalized, enhanced care to patients with cognitive impairments. You will oversee referrals, assess patient needs, and strategically allocate team members to ensure the best possible care.

You will be at the forefront of our mission to meet and exceed the National Dementia Strategy and NICE Guidelines. Collaborating with a wide range of professionals and stakeholders, you will elevate the quality of dementia care across the trust, ensuring that our practices are cutting-edge and compassionate.

Deliver Clinical Excellence:

As an expert in dementia and delirium care, you will provide invaluable guidance and support to multidisciplinary teams, shaping the future of dementia care within our hospital and beyond. Your expertise will help us achieve national standards, as set out in the National Dementia Strategy and the NICE Guidelines for dementia.

The Dementia Clinical Nurse Specialist will work closely with the Lead Nurse for Dementia to deliver evidenced-based practice and implement the outcome recommendations of the National Dementia Strategy, NICE guidance, National Dementia Audit, and the Dementia Friendly Hospital Charter (PLACE).

You will provide expert advice and support to professionals involved in the care of patients with dementia and delirium, contributing towards service improvement in these areas, namely by meeting the NICE Dementia Quality Standards.

The role is currently hospital-based but will develop significant links with existing community teams, ensuring seamless care transitions and improved patient outcomes.
