Job Overview

This is a challenging and rewarding role that requires a strong understanding of community services and holistic care needs. As a Flexible Worker, you will work closely with clients, healthcare professionals, and other agencies to ensure all client needs are met.

Responsibilities
  1. Conduct comprehensive assessments to inform treatment journeys.
  2. Referral and partnership work with other agencies, including training and employment services.
  3. Collaboration with healthcare professionals to monitor and manage client health and minimize risks of harm.
  4. Motivating clients to engage with identified support services and care plans.
  5. Supporting clients in achieving their care plan goals, including referrals to community services, housing advice, benefits guidance, and training opportunities.
