About the Role

We are seeking a highly skilled and motivated Occupational Therapist to join our Community Rehabilitation Team. As a key member of our team, you will be responsible for providing patient-centered care to individuals with long-term and acute conditions, falls prevention, and care of the older person. You will work autonomously, as part of a team, to provide support and rehabilitation to patients in their homes, nursing and residential homes.

Key Responsibilities

• Provide high-quality, patient-centered care to individuals with long-term and acute conditions
• Conduct thorough assessments to identify patients' needs and develop personalized rehabilitation plans
• Collaborate with healthcare professionals and community teams to ensure seamless care delivery
• Develop and maintain effective relationships with patients, families, and caregivers to promote patient-centered care
• Participate in the development and implementation of community rehabilitation programs to improve patient outcomes
• Contribute to the education and training of colleagues on best practices in community rehabilitation
• Engage in quality improvement initiatives to enhance patient care and services

Requirements

• Bachelor's degree in Occupational Therapy
• Current registration with the Health and Care Professions Council (HCPC)
• Proven experience in community rehabilitation or a related field
• Excellent communication and interpersonal skills
• Ability to work autonomously and as part of a team
• Strong problem-solving and critical thinking skills
• Commitment to providing high-quality patient-centered care
• Ability to adapt to changing priorities and deadlines
