Day Program Specialist

We are seeking a highly motivated and compassionate Day Program Case Manager to join our team at Communitas, Inc. As a key member of our Day Program Services, you will work closely with individuals, families, and other stakeholders to ensure seamless coordination of services.

Key Responsibilities:

  • In conjunction with other departments, ensure that services meet individual needs and promote individual choices.
  • Provide guidance to department staff in completing required documentation.
  • Participate in planning meetings (team meetings, DHSP, ISP) to advocate for individuals' needs.

Requirements:

  • High School diploma required (BA degree in Human Services or related field preferred).
  • At least 1 year of experience supporting adults with developmental disabilities is preferred.
  • A passion and dedication to supporting our people is essential.
