Adult Advanced Practitioner

We are seeking an experienced and skilled Adult Advanced Practitioner to join our team in the Frailty Unit at Swansea Bay UHB. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with frailty syndromes, injuries, illnesses, and acute exacerbations of chronic conditions.
Main Responsibilities:- Assess, diagnose, and manage patients autonomously and as part of the MDT.
- Work collaboratively with geriatricians, nurses, AHPs, and pharmacists to provide targeted care to vulnerable patients.
- Develop and implement effective care plans to improve patient outcomes and reduce hospital-acquired harms.
- Contribute to the development of clinical practice and policy within the Frailty Unit.
- Registered Practitioner (e.g. NMC/HPC)
- Accredited as an Advanced Practitioner
- Willingness to undertake further Masters-level education relevant to the role.
- Evidence of higher-level practice development within a specialist area.
- Knowledge of Safeguarding Adults/Children, including the Mental Health Act and Mental Capacity Act.
- Knowledge of quality and service improvement methodologies.
- Ability to critically analyze research and apply it to practice settings.
- Leadership and change management skills.
- Able to travel between sites.
- Able to work hours flexibly.
- Welsh Speaker.
