Key Responsibilities- Build strong relationships with families and young people to understand their needs and develop tailored support plans.
- Work with statutory services to assess support needs and create bespoke plans to meet those needs.
- Develop and implement move-in plans to ensure a smooth transition into their new home.
- Drive the execution of support plans, removing barriers and helping to solve problems.
- Offer practical and emotional support to individuals and families as needed.
- Facilitate neighbourhood projects to promote social outcomes and community engagement.
- Professional qualification in social work, youth work, health, or a related field.
- Previous experience working with children and families, with a sound knowledge of safeguarding procedures.
- Experience of complex case management and delivering to assessment and outcome-based frameworks.
- Understanding of welfare rights and providing money advice.
