As a Care Specialist, you will be responsible for providing one-to-one support to clients, enabling them to live independently in their own homes. **Key Responsibilities:

  • Delivering person-centered care services to clients in their own homes
  • Assessing client needs and developing individualized care plans
  • Maintaining accurate records and reporting any concerns or changes in client condition
  • Working collaboratively with multidisciplinary teams to ensure seamless service delivery
