Community Reablement Specialist

This is an exciting opportunity for a highly skilled Occupational Therapist to join our Reablement Team in Wiltshire, where you will have a significant impact on the lives of individuals following illness, injury, or loss of confidence.
About the Job:
- Conduct comprehensive assessments to identify strengths and needs of customers with complex occupational requirements.
- Work collaboratively with customers, families, and healthcare professionals to develop personalized reablement plans.
- Provide expert guidance on adaptations, equipment, and strategies to promote safe and independent living.
- Take an active role in supervising and supporting colleagues to ensure excellent practice and evidence-based care.
Responsibilities:
- Develop and implement creative reablement plans to enable customers to achieve their goals, promoting greater independence and well-being.
- Collaborate with other healthcare professionals to provide holistic support and address complex needs.
- Stay up-to-date with best practices through ongoing professional development, including attendance at statutory and non-statutory training.
Requirements:
- Qualification in Occupational Therapy from a recognized institution.
- Experience in managing complex cases and providing occupational therapy assessments.
- Able to travel daily and work in various settings, including customers' homes or hospitals.
