About the Role
We are seeking a skilled and dedicated Community Staff Nurse to join our team at Dorset Clinical Commissioning Group. As a Community Staff Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.
Key Responsibilities
- Provide comprehensive nursing care to patients in their own homes, including assessments, treatments, and interventions.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct regular reviews and assessments to ensure patients are receiving the best possible care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in education and training programs to stay up-to-date with best practices and evidence-based care.
Requirements
- Registered practitioner to degree/diploma level.
- Membership of the relevant Professional Body.
- Learning and Assessing in Practice Qualification or equivalent practice assessors training.
- Evidence of recent professional development in an up-to-date portfolio.
- Able to communicate effectively at different levels of the organization and with staff, patient/service users, visitors, or external organizations both verbally and in writing.
- Ability to use own initiative within the sphere of authority.
- Willingness to advance own clinical knowledge, skill, and competence based on current evidence.
- Able to analyze and assess situations and to interpret potentially conflicting situations and determine appropriate action.
Desirable Criteria
- Recent previous experience within a comparable role.
- Advanced wound care experience, end of life care.
