Job Title: Nurse Case Manager

Job Summary:
We are seeking a skilled Nurse Case Manager to join our team at Routes Healthcare (North). As a key member of our care team, you will be responsible for managing complex care packages and providing high-quality clinical care to our clients.

Key Responsibilities:
Develop and implement care plans that meet the unique needs of each client
Manage a caseload of high dependency patients and provide clinical leadership to the care team
Collaborate with multidisciplinary teams to ensure seamless care delivery
Conduct regular assessments and reviews to ensure the best possible outcomes for clients

Requirements:
RN Qualification (Adults)
Valid NMC pin
Experience in a high dependency or critical care setting
Strong communication and interpersonal skills
Ability to work autonomously and as part of a team
