Sheffield Health & Social Care NHS Foundation Trust is seeking a highly skilled Community Practitioner to join their team. This role offers the opportunity to make a real difference in the lives of individuals and communities in Sheffield.

About the Role

This is a unique partnership role that involves working across various organizations, including Heeley Trust, Heeley Plus Primary Care Network, and Sheffield City Council. As a Community Practitioner, you will be working to support the development of the Gleadless & Heeley Neighbourhood Mental Health Centre, acting as a key liaison and outreach professional between the centre, teams working at the centre, and wider communities.

Key Responsibilities
  • Work with colleagues, partner organizations, service users, and their carers to ensure co-production in terms of scope, planning, design, and delivery of workstreams/projects within the programme.
  • Support the delivery of coproduced care pathways through the workstream projects to a high standard and in a cost-effective manner.
  • Provide consultation and training to staff in understanding and working with people with complex social, emotional, and interpersonal difficulties.

The ideal candidate will have experience of leading the co-design of project work, managing projects, and building relationships with multidisciplinary colleagues. A degree-level qualification or equivalent experience is essential. The salary for this role is approximately £35,000 - £45,000 per annum, depending on experience.
