Job Title: Advanced Nurse Practitioner

Are you a skilled and experienced Advanced Nurse Practitioner looking for a new challenge? We have an exciting opportunity for a Home Visiting Advanced Nurse Practitioner to join our team in the North West.

Job Summary:

We are seeking a highly motivated and autonomous Advanced Nurse Practitioner to work within our Home Visiting Service. As a key member of our team, you will be responsible for delivering evidence-based nursing services to patients in their homes. You will work closely with our management team to review clinical policy and procedure, and develop, implement, and embed health promotion and wellbeing programmes.

Key Responsibilities:

  • Deliver routine nursing care to patients in accordance with clinical based evidence, NICE, and the NSF.
  • Work autonomously as a Nurse Prescriber, responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, and well man clinics.
  • Support the management team in reviewing clinical policy and procedure.
  • Develop, implement, and embed health promotion and wellbeing programmes.
  • Evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions.
