We are seeking an experienced and skilled Band 5 Community Hospital Staff Nurse to join our team at Wye Valley NHS Trust. This full-time position is based at Belmont and involves working with patients registered with Primary Care Networks across Herefordshire.
About the TeamYou will be part of a dynamic and supportive team that provides high-quality patient care in a community setting. Our team is dedicated to delivering person-centered care and making a positive impact on the lives of our patients and their families.
As a Community Nurse, you will work closely with other healthcare professionals to provide comprehensive care that meets the needs of our patients. You will also be responsible for planning and delivering care that supports patients towards self-management and enables them to live fulfilling lives.
We are committed to providing training and support to help you succeed in this role. With a caseload of around 850 patients, you will benefit from a supportive team environment and opportunities for professional development.
Requirements- Registered Nurse Level 1 (Adult) qualification
- Evidence of continued professional development
- IT literacy and willingness to learn new systems
- Excellent communication and interpersonal skills
