About the Role

We are seeking a highly motivated and empathetic Locality Care Worker to join our team. In this role, you will work closely with carers and community partners to deliver targeted support and interventions that promote independence and wellbeing.

Key Responsibilities
  • To work collaboratively with carers and community partners to identify and address specific needs and concerns.
  • To develop and deliver tailored support plans that empower carers to make informed choices and maintain their independence.
  • To engage in outreach work, promoting our services and facilitating peer support groups.
Requirements
  • A genuine passion for working with carers and a deep understanding of their unique challenges.
  • Proven experience in delivering targeted support and interventions to adults.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with carers and colleagues.
