About the Role

We are seeking a highly skilled and compassionate Community Support Specialist to join our team at Swansea Bay University Health Board. As a key member of our community healthcare team, you will play a vital role in delivering high-quality patient care and support in a community setting.

Key Responsibilities
  • Carry out delegated clinical and care tasks to a high standard, under the direction and supervision of a Registered Practitioner/Assistant Practitioner.
  • Work autonomously and as part of a multi-disciplinary team to provide patient-centered care and support.
  • Communicate effectively with patients, relatives, and carers, and overcome barriers to effective communication in a sensitive and respectful manner.
  • Recognize and report any issues related to safeguarding vulnerable adults/children in a timely manner.
  • Contribute to the development and implementation of care plans that promote patient independence and well-being.
Requirements
  • Minimum of Qualification Credit Framework - QCF Level 3 (formerly NVQ Level 3)
  • Evidence of maintaining personal development and understanding of the scope of the role in the context of the nursing team.
  • Evidence of working within a specialist care setting.
  • Knowledge of fundamental aspects of care and the importance of promoting health and wellbeing.
Desirable Criteria
  • Understanding of caring for individuals with particular conditions, such as dementia, mental illness, or learning disabilities.
  • Ability to speak Welsh (desirable).
