The Role: Hospital Discharge and Assessment Specialist

We are seeking a highly motivated and compassionate individual to join our dedicated team of professionals in the Hospital Discharge and Assessment department. In this role, you will play a crucial part in ensuring smooth transitions for patients leaving hospital and supporting their successful reintegration into their homes and communities.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs upon discharge from hospital.
  • Develop personalized care plans to facilitate a safe and effective transition home.
  • Coordinate with healthcare professionals, social workers, and other stakeholders to ensure seamless patient care.
  • Provide ongoing support and guidance to patients and their families during the post-discharge period.
