Job Summary

As a Social Work Officer, you will play a vital role in supporting residents to improve their independence and wellbeing by working closely with partners and maximizing the use and benefit of community resources.

Key Responsibilities
  • Conduct strengths-based assessments, reviews, and support planning to enable residents to maximize their independence, improve quality of life, and build their resilience.
  • Provide practical support to residents and carers, including accessing living aids, navigating the welfare benefit system, and providing advice and information to service users.
  • Assist in arranging short-term care and providing clients and carers with respite to reduce carer breakdown.
  • Work with adult clients, relatives, carers, and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to support planning.
  • Liaise with colleagues across the health and social care partnership to gather information relevant to assessment and support planning activities.
  • Maintain and update reliable, timely, and accurate case notes, systems, and other records, and write reports as required.
  • Undertake reviews of care packages and contribute to effective Multi-Disciplinary Team working by inputting through meetings and working with others to achieve best outcomes for those accessing services.
  • Attend safeguarding strategy meetings as required.
  • Complete Continuing Health Care checklists and identify those that require full assessments.
Requirements

To be successful in this role, you will need to have a strong understanding of social care principles and practices, excellent communication and interpersonal skills, and the ability to work effectively in a team environment.
