We are seeking a dedicated and experienced RGN Qualified Nurse to join our team as a Community Nurse Case Manager. This role offers a unique blend of home-based work and community visits, allowing you to make a real difference in the lives of those with complex care needs.
About the Role:- Conduct initial assessments in the community to ensure clients receive the best possible care.
- Provide clinical oversight to your team, ensuring high-quality care is delivered consistently.
- Collaborate with the wider multidisciplinary team (MDT) to ensure seamless care coordination.
- Attend meetings and make necessary referrals to ensure clients receive the support they need.
- Manage a team of non-clinical case managers to ensure effective care delivery.
- Annual salary of £48,000
- Bonus opportunities
- Full-time, permanent position
- Monday to Friday, 9am-5pm with some flexibility
- Two days home-based, three days community visits
- Travel across the Midlands
- RGN qualification with excellent clinical skills in complex care
- Experience with peg feeds, ventilators, and tracheostomy management
- A valid driving licence, as the role involves remote and community-based work
- Will live in or around the Nottingham area
