Health and Social Care Manager

We are looking for a dedicated Complex Care Coordinator to join our team at Brook Street Social Care NMR. In this role, you will be responsible for delivering high-quality care packages to individuals with medium to complex care needs.
About the Role:
- This is an exciting opportunity to join a forward-thinking provider committed to delivering person-centred care.
- You will work closely with clinicians and external providers to review care packages and ensure they meet company and CQC standards.
- You will also prepare and send monthly reports to the Clinical Commissioning Group (CCG).
Responsibilities:
- Implementing new care packages and allocating staff to match client needs.
- Maintaining regular contact with service users and updating care plans accordingly.
- Ensuring person-centred care is delivered effectively and care plans are updated regularly.
Requirements:
- Proven experience in a similar role.
- Relevant care qualification (level 3 in health and social care) or willingness to work towards this.
- Excellent communication skills, attention to detail, and strong organisational abilities.
- Full UK driving licence, access to a vehicle, and flexibility to travel throughout the West Midlands.
