Community Healthcare Associate

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a highly motivated and compassionate individual to join our Urgent Community Response Team as a Healthcare Support Worker. In this role, you will work collaboratively with our team to provide high-quality, patient-centred care to individuals in need, promoting health and wellbeing through effective communication and documentation.
As a Healthcare Support Worker, you will be responsible for maintaining excellent communication with patients, relatives and colleagues, using a range of verbal and non-verbal communication tools to convey complex information effectively. You will also be responsible for documenting patient care in a clear, concise and legible manner, reflecting on barriers to communication and using knowledge of different methods to support and improve communication with individuals.

Requirements:
Essential qualifications include a Full Driving Licence, experience within a healthcare setting and literacy and numeracy level 2 (GCSE A-C).
