District Nurse/Case Manager

We are seeking a highly skilled and experienced District Nurse/Case Manager to join our dynamic Community Nursing Team. As a key member of our team, you will be responsible for providing high-quality, patient-centered care to housebound patients in their own homes, promoting independence and preventing unnecessary hospital admissions.
The successful candidate will have a strong background in community nursing, with a focus on holistic care and patient assessment. You will be expected to work independently, lead and be part of a team, and demonstrate excellent communication and interpersonal skills.

Main Duties and Responsibilities- Assess and manage the health needs of an identified population, developing and implementing programs of care to target groups with specific health needs.
- Provide specialist advice and guidance to patients, carers, relatives, and other agencies, promoting health and social care.
- Participate in the development, training, and mentorship of pre-registration students and medical students, as well as staff induction and preceptorship programs.
- Contribute to the implementation and audit of policies, guidelines, and procedures, ensuring evidence-based practice and patient-centered care.
- Supervise and direct work undertaken by other members of the skill mix team, ensuring effective communication and continuity of care.
- Registered Nurse (1st Level) qualification.
- Degree-level District Nursing Specialist Practitioner Qualification (or working towards).
- Leadership Qualification.
- Coaching Skills.
- Significant post-registration community experience.
- Competent End of Life experience.
- Experience of Personalised Care Planning and complex patient management.
- Experience of working in a team environment.
- Experience of nurse leadership and management.
- Evidence of Continued Professional Development.
- Experience in managing complaints.
- Experience in service improvement.
- Ability to work independently, lead, and be part of a team.
