Key Responsibilities

This role plays a vital part in supporting GPs and the broader primary care team to deliver safe and effective patient care.

The successful candidate will collaborate closely with the GP and other practice clinical and non-clinical staff, providing medication-related patient services and direct patient care. They will also guide and support the practice team to ensure safe and effective prescribing.

Main duties include consulting with patients who have one or more medical conditions requiring medication optimization. This involves reviewing ongoing medication necessity, assessing monitoring needs, and providing support to patients in managing their medications for optimal outcomes.

  • Consult with patients on medication optimization for conditions like COPD, asthma, and hypertension.
  • Review and provide recommendations on medication changes.
  • Conduct clinical medication reviews with patients and offer suggestions to senior clinical pharmacists, nurses, and/or GPs.
  • Offer solutions and recommendations to GPs, practice staff, and healthcare teams.
  • Follow up with patients to monitor medication effects.

A key aspect of this role is implementing changes to medications in response to MHRA alerts, product withdrawals, and national guidelines. This includes monitoring practice prescribing compliance with local formularies and recommending medication choices for hospital doctors or shared care.
