About You

Key Responsibilities
  • To assess the needs of vulnerable adults and develop personalized care plans that meet their needs and preferences.
  • To work with individuals, families, and communities to achieve positive outcomes and improve the quality of life for vulnerable adults.
  • To provide support and guidance to individuals and families to help them access the services and resources they need.
  • To work collaboratively with other agencies and organizations to ensure that vulnerable adults receive the support and services they need.
