Job Description: Discharge Planning Specialist

As a Discharge Planning Specialist at Whittier Rehabilitation Hospital Bradford, you will be responsible for ensuring timely and effective discharges for our patients. This includes collaborating with the healthcare team, patient, and family to identify suitable next steps, making referrals to appropriate agencies or facilities, and coordinating transfers to maintain continuity of care. Your expertise in case management, utilization review, and insurance principles will be invaluable in achieving optimal patient outcomes.

About the Ideal Candidate:
We are looking for a highly organized and communicative individual with a minimum of 5 years of varied nursing experience, preferably in orthopedic, med-surg, geriatric, or rehabilitation settings. A strong understanding of insurance principles, including eligibility, benefits, and limitations, is essential. The ability to assess, analyze, plan, implement, and evaluate is critical in this role. Strong problem-solving skills, attention to detail, and a commitment to delivering exceptional patient care are also required.

Key Responsibilities:
- Collaborate with the healthcare team, patient, and family to establish a treatment plan with realistic goals and a target date to achieve the goals.
- Make referrals to the next level of care and coordinate the transfer to maintain continuity of services.
- Maintain patient privacy and confidentiality at all times, releasing only authorized information to relevant parties.
