Main Responsibilities- Deliver patient care in patients' own homes according to an agreed programme of care and care plan.
- Conduct an accurate holistic assessment of patients' needs in accordance with approved models of care.
- Safely administer a range of therapeutic interventions relevant to NHS projects and training as per agreed clinical pathways within scope of practice.
- Assist patients in their activities of daily living and support multi-disciplinary goals of optimising patients' level of independence.
- Chair/attend all relevant committees aligned to the remit of the role.
- Registered Sick Children's Nurse.
- Competent in the administration of IV therapies.
- Strong communication skills, including actively listening, communicating sensitively, and putting people at ease while making technical details easy to understand.
