About the Role

Your Responsibilities

  • You will co-ordinate shifts to ensure seamless care delivery, collaborating with healthcare professionals to implement bespoke plans of support.">
  • You will work closely with patients, families, and carers to understand their unique needs and preferences, promoting person-centred approach and outcome-based support planning.">
  • You will contribute to the admissions and discharge process, ensuring a smooth transition for service users.">
  • You will participate in staff supervision and development, fostering a culture of continuous learning and improvement.">

Your Qualifications and Experience

">

We are seeking individuals with a Foundation Degree or Higher Education Certificate in Integrated Care or a related field, or those who have completed a Level 3 qualification in health or social care with a willingness to work towards level 4. A minimum of 2 years' proven experience in a health and/or social care environment is required, with a strong ability to work effectively as part of a team.

Salary and Benefits

The successful candidate will be rewarded with an estimated salary range of £24,645 - £27,202 per annum, pro-rata. Our comprehensive benefits package includes optional membership to a contributory pension scheme, lease cars, and extensive opportunities for professional development.
