About the Role

We are seeking a skilled and compassionate Social Worker to join our Adult Central South Neighbourhood team in Bolton. As a key member of our team, you will work closely with health colleagues to support adults and carers eligible for assessment and care under the Care Act 2014.

Key Responsibilities:

  • Conduct assessments and care planning to ensure the welfare and assessed needs of vulnerable adults are managed in a safe and inclusive way.
  • Participate in Best Interests meetings and handle complex cases with sensitivity and expertise.
  • Collaborate with Social Workers, Occupational Therapists, Nurses, and Mental Health Practitioners to deliver person-centred, strength-based care.
  • Coordinate resources from social care, health, and other agencies to support independence and safeguarding.
  • Complete administrative tasks, attend case conferences, and represent the Council in court when necessary.

Requirements:

  • Hold a degree in social work and be registered with Social Work England.
  • Have evidence of continuing professional development and a strong understanding of the application of Care Act, Mental Capacity Act, and Adults Safeguarding.
  • Be a confident decision maker and able to stay calm under pressure.
  • Demonstrate commitment and enthusiasm to working closely with vulnerable adults, their families, and carers, as well as other professionals and managers.
