We are seeking an experienced Advanced Nurse Practitioner to join our team at Brook Medical Centre. As a key member of our primary care team, you will play a vital role in delivering high-quality, patient-centred care to our patients.
Main Responsibilities- Develop and lead health promotion and wellbeing programmes to support the health and wellbeing of our patients.
- Manage acute and chronic medical conditions, creating and evaluating treatment plans to ensure the best possible outcomes for our patients.
- Provide advanced nursing care, wound care, and specialist clinics, including contraception, ear syringing, and travel medicine.
- Request and interpret pathology tests, and maintain accurate clinical records to ensure the highest standards of care.
- Prescribe medications, deliver immunisation programmes, and manage mental health needs to support the overall health and wellbeing of our patients.
Brook Medical Centre serves 15,000 patients across two sites in Bradeley and Chell, Stoke-on-Trent. Our team of almost 50 healthcare professionals is dedicated to providing high-quality, accessible primary healthcare services to the underserved population in our area.
