About the Role

We are seeking a skilled Home Health Assistant to join our team at Dorset Healthcare University NHS Foundation Trust. As a member of our Multidisciplinary team, you will be working in the community, providing rehabilitation and care services to patients in their own homes.

Your primary responsibility will be to work with patients who require therapy to prevent hospital admission and facilitate earlier discharge. This role involves treating patients in their own homes, promoting independence, and enabling them to achieve maximum strength, confidence, and function following illness or surgery.

The successful candidate will have experience in rehabilitation or a caring role, preferably in both ward and community settings, as well as good verbal, written, and team-working skills. You will also need to be flexible and confident when working alone.

As a Home Health Assistant, you will be required to drive a car and visit patients in their homes and other community settings under the close supervision of clinicians within the team.

Key Responsibilities
  • Providing rehabilitation and care services to patients in their own homes
  • Working collaboratively with multidisciplinary teams to achieve patient goals
  • Developing and implementing individualized care plans to promote patient independence

Salary: £25,791 - £32,878 per annum (inclusive of Inner London Weighting)
