Main Responsibilities:- Assess patient needs and develop personalized care plans to ensure safe and effective care.
- Lead and support a team of healthcare professionals to provide comprehensive care to patients with complex needs.
- Collaborate with other healthcare agencies to ensure seamless care coordination and patient outcomes.
- Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
