About the Role

We are seeking a skilled and dedicated Community Health Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care coordination.

Main Responsibilities
  • Assess patient needs and develop personalized care plans to meet their unique requirements.
  • Provide expert nursing care to patients in their own homes, promoting independence and dignity.
  • Work collaboratively with other healthcare professionals to ensure comprehensive care coordination.
  • Lead and support staff to deliver high-quality care, promoting a positive and inclusive team culture.
Requirements
  • Minimum of two years' experience in a community nursing team.
  • Primary care experience and a strong understanding of community health needs.
  • Ability to effectively manage challenging behavior and conflict.
  • Full UK driving license and access to a car for work purposes.
  • Evidence of ongoing professional development and a willingness to undertake further qualifications/training.
