About the Role

Norfolk Primary Care C.I.C. is seeking an experienced Advanced Nurse Practitioner to join our Home Visiting Service team. As an Advanced Nurse Practitioner, you will provide high-quality care to housebound patients on behalf of their registered GP Practice and the wider PCN.

Key Responsibilities

You will be responsible for working with all HVS participating Practices, focusing on proactive support to patients residing at home, including care homes. The aim of the HVS is to reduce the number of avoidable admissions into hospital and deliver more consistent, enhanced services from Primary Care.

Requirements

To be successful in this role, you will need to be an experienced clinician who can act within their professional boundaries and competencies. You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and follow-up of care.
