About the Role:
We are seeking an experienced and compassionate Staff Nurse to join our Patient and Carer Wellbeing Service team at The Myton Hospices. As a Palliative Care Specialist, you will play a vital role in providing high-quality care and support to patients and their families living with life-limiting illnesses.
Key Responsibilities:
- Coordinate the assessment, planning, and evaluation of individualized patient-centered care, with a holistic approach that is sensitive and responsive to the ever-changing needs of patients, families, and carers.
- Work closely with a multi-disciplinary team to ensure that care and support reflect individual needs, and to alert the Service Lead/Senior Nurses of any concerns or crises.
- Proactively work in partnership with other providers externally to ensure that all information pertaining to the patient's condition and care needs are communicated effectively.
- Act as a positive role model to others and ensure that practice of specialist palliative care is evidence-based.
Requirements:
- Significant post-registration experience in palliative and end-of-life care.
- Experience of needs assessment/care planning, multidisciplinary team working, and caring for patients and families with life-limiting/life-threatening conditions.
- Good knowledge of assessment and symptom management.
- Excellent personal and communication skills, teaching and assessing skills, and organizational ability.
- Motivated and enthusiastic, with a commitment to the Myton values.
