About the Role

We are seeking a highly skilled and experienced Adult Advanced Practitioner to join our team at Stalybridge Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient-centred care to our population.

Key Responsibilities
  1. Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patient's own home, or other environments where patient care is carried out.
  2. Refer patients directly to other services/agencies as appropriate, utilising Practice and local guidelines and in a timely manner.
  3. Pro-actively identify, diagnose, monitor, and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.
  4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
  5. Support patients to adopt health promotion strategies that promote healthy living and to apply principles of self-care.
  6. Recognise, assess, and refer patients with mental health needs as appropriate.
  7. Communicate with and support patients receiving bad news.
  8. Support the Covid vaccination programme.
  9. Participate in planning and implementing changes within the area of care and responsibility.
  10. Contribute and participate in the development of local guidelines, protocols, and standards.
  11. Ensure the principles of infection control and local/national policies and guidelines are applied throughout the practice, monitoring and implementing changes as required.
  12. Clinically oversee the PCN Health & Wellbeing team, for example, social prescriber, cancer care coordinator, Complex Care team.
Requirements
  • Registered Nurse (NMC) or Registered Paramedic (HCPC).
  • Independent non-medical prescriber proven ability to use Patient Group Directions.
  • Ability to assess and manage patient risk effectively and safely.
  • Demonstrable evidence and commitment to professional development.
  • Knowledge of health promotion strategies.
  • Knowledge of Quality and Outcomes Framework.
