Job summary

At Stalybridge Primary Care Network (PCN) we are looking to expand our already established paramedic department with an opportunity to offer a motivated paramedic the chance to join our team.

 As a General Practice PCN Paramedic, you will work aspart of a wider Primary Care team, including GPs, Advanced Nurse Practitioners, Mental HealthPractitioners, Practice Nurses and Healthcare Assistants across 9 practices inthe PCN, covering a population of around 42,000. The post holder will provide a high-quality, cost-effective service tothe patients of the Stalybridge, Mossley and Dukinfield PCN undertaking daily acute and routine home visits, conducting examinations and diagnosing andprescribing treatments and medication (if prescriber) while working withindefined procedures and protocols as laid down by the GPs.

Main duties of the job

The post holder will be responsible for carrying out consultations to patients in practice sites or the central PCN Hub and for visiting patients in their place ofresidence who present with a wide range of minor illness, urgent and same dayrequests for treatment and chronic disease management; providing care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.

The post holder will need to demonstrate safe, clinical decision-makingand expert care, including assessment and diagnostic skills. There will be theneed to prioritise and triage the needs of patients accordingly making anynecessary referrals for investigations in the appropriate manner.

These are groups of GP practices are working in partnership tosupport their local population. The network covers a population of nearly 42,000 patients and has recently opened a brand new central PCN hub right in the town centre.

Clinical responsibilities

The paramedic will work as part of a multi-disciplinary team (MDT) within the PCN.

They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.

They will be able to:

Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance Perform and interpret ECGs Perform investigatory procedures as required, and Undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance. Support the delivery of anticipatory care plans and lead certain services ( monitoring blood pressure and diabetes risk of elderly patients). They will provide an alternative model to urgent and same day GP home visit for the network. Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.

Communication

Liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways. Maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision. Ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission. Take an active role in maintaining expert communication with all patients, carers and relatives which meets their individual needs. Deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily in accordance with local protocols. Communicate effectively in demanding and challenging situations, where there may also be hostility.

Professional

Input to and extract information from clinical IT system. Work collaboratively with other members of the health care team such as the, staff in Care Homes, Ambulance Service, and Secondary Care etc. Ensure that the wellbeing of patients and staff is maintained in accordance with Practice and PCN policies. To undertake clinical audits in line with the organisations clinical audit plan, and to participate as required in equipment testing and other research. Contribute to the analysis of information to inform future development of the service. Participate and contribute to clinical meetings. Identify risks to service provision and highlight them to the Clinical Lead. Work within the requirements of Data Protection legislation as it applies to patient records. Adhere to HCPC Code of Professional Conduct and at all times practice within the scope of professional practice Maintain current professional registration and indemnity.

Organisational

Operate as an effective member of the practice team and the PCN. Maintain accurate and systematic documentation of patient interactions and clinical care provided, including any adverse events, child protection issues or other notifiable matters. Provide information to inform the evaluation and efficiency of the role and for performance management purposes as requested. Engage in self appraisal and utilise reflective practice to enhance and develop personal and organisational practice. Assist in marketing and promoting the role and organisation within the health community both locally and nationally.

Managerial

Ensure effective time management and prioritisation of workload. Ensure the security, safe storage and maintenance of any allocated equipment and goods. Ensure practice is within good clinical governance and based on sound up-to- date clinical evidence. Where appropriate, to identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager Person Specification

Experience

Essential

- 3 years working as band 6 or equivalent paramedic - perform specialist health checks and reviews within their scope of practice and in line with local and national guidance - perform and interpret ECGs - perform investigatory procedures as required, and undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance.

Qualifications

Essential

- educated to degree/diploma level in Paramedicine or equivalent experience - registered with the Health and Care Professions Council (HCPC) has completed their two-year Consolidation of Learning period as a newly qualified paramedic - has a further three years experience as a band 6 (or equivalent) paramedic - is working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement of reimbursement for that individual (or a longer time period as agreed with the commissioner), has completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.

Desirable

- Experience working in a Primary Care setting - Experience using clinical systems such as EMIS and TPP SystmOne - Non Medical Prescribing level 7 Qualification - or willing to work towards.
