Polish Speaking Complex Needs Navigator

We are seeking a highly skilled and compassionate Polish-speaking Complex Needs Navigator to join our team at Change Grow Live. As a key member of our Rough Sleepers Drug & Alcohol Team (RSDAT), you will play a vital role in engaging with Polish nationals who are rough sleeping or at risk of rough sleeping through street-based outreach.
Key Responsibilities- Develop and implement personalized action plans to help clients engage with appropriate services and move away from street-based living.
- Carry a small caseload of complex clients, assessing their needs and developing strategies to support their recovery.
- Co-facilitate Polish-speaking groups for individuals at different stages of change, providing a safe and supportive environment for them to share their experiences and receive guidance.
- Work closely with partner services to ensure action plans are carried out and share relevant information to support the client's journey.
- Maintain accurate and confidential service user records, adhering to Change Grow Live's procedures and local authority requirements.
We are looking for an individual with a strong background in engaging with individuals with multiple and/or complex needs. You will have a determined approach to finding solutions and be fluent in Polish, with the ability to communicate effectively with Polish-speaking service users.
