Job Summary

We are seeking a skilled and compassionate Social Care Assistant to join our team at Corus Consultancy. As a Social Care Assistant, you will play a vital role in supporting vulnerable adults and informal carers, providing assessments, monitoring, and reviewing their social care needs.

Key Responsibilities
  • Conduct assessments and reviews of social care needs, identifying solutions and making referrals to external agencies as necessary.
  • Manage a caseload of varying complexity, working under the supervision of a qualified senior practitioner.
  • Take responsibility for casework, promoting independence and assessing and managing risk.
  • Conduct Assistive Technology assessments and arrange provision of suitable equipment.
  • Conduct risk assessments and undertake initial enquiries in response to vulnerable adults safeguarding concerns.
  • Contribute to the Multi-Disciplinary Support Team, undertaking Continuing Health Care Checklists.
  • Assess Mental Capacity and make Best Interest decisions when required, in accordance with the Mental Capacity Act.
Requirements

To be successful in this role, you will need to have a strong understanding of the Care Act and the Mental Capacity Act, as well as excellent communication and interpersonal skills. You will also need to be able to work independently and as part of a team, with a high level of discretion and confidentiality.
