Job Title:

Community Healthcare Professional - Rapid Response Team

About the Role:

We are seeking a skilled and experienced Community Nurse to join our Rapid Response Team as a Senior Community Nurse. The successful candidate will be responsible for delivering high-quality, skilled nursing care to patients in their homes and providing rapid response, rehabilitative, and post-acute care in various community settings.

Key Responsibilities:
  • Provide skilled, effective, and evidence-based nursing care to patients in community settings.
  • Use enhanced skills and knowledge to assess physical and psycho-social needs and develop comprehensive and concordant care plans with patients, carers, and the Multidisciplinary Team.
  • Maintain working relationships with staff across the North Division, unplanned care team, and integrated locality teams.
  • Work within Clinical Guidelines, Policies, Procedures, and utilise expert judgment to work outside these frameworks where necessary.
Person Specification:

The ideal candidate will have:

  • A Registered Nurse qualification with current NMC registration.
  • Evidence of recent and continuing post-registration professional education and training.
  • Extensive range of clinical skills and experience, including IV therapy, venepuncture, cannulation, rehabilitation, and working with people with complex health needs.
  • Experience of supervising junior staff and coordinating daily activities.
  • Basic IT skills, including use of Microsoft Office package and email.

Estimated Salary:
