About the Role">

We are seeking a dedicated Social Worker to join our multidisciplinary team at South West London & St Georges Mental Health NHS Trust. As a key member of the Hume ward team, you will work closely with patients who require short- and long-term hospital admissions due to severe mental health issues.

The ideal candidate will have a strong background in social work practice, particularly in forensic settings, and possess excellent communication and assessment skills. You will be responsible for conducting comprehensive assessments, compiling detailed social histories, and contributing to treatment plans that address the complex needs of our patients.

Key Responsibilities
  • To conduct thorough assessments of patient needs, identifying areas for intervention and developing targeted care plans.
  • To collaborate with multidisciplinary teams, including healthcare professionals, therapists, and other specialists, to ensure cohesive and effective patient care.
  • To maintain accurate and up-to-date records, adhering to trust policies and procedures.
Person Specification
  • A recognised UK social work qualification (e.g., Social Work Degree, CQSW, CSS, or Dip SW) and current eligibility for registration with Social Work England.
  • Post-qualifying training in relevant areas of social work and/or mental health, with evidence of continuing professional development.
