Job Overview

We are seeking a skilled and compassionate Nursing Care Professional to join our Community Nursing Team in the Tweeddale locality. This is an exciting opportunity to work with a dynamic and supportive team, providing high-quality care to patients in their own homes and community settings.

Key Responsibilities

  • Assess patient needs and develop personalized care plans to meet their unique requirements.
  • Deliver high-quality nursing care, working closely with patients, families, and multidisciplinary teams to achieve optimal health outcomes.
  • Contribute to the development of new care pathways and services, ensuring they are patient-centered and meet the needs of our communities.
  • Collaborate with colleagues to share best practice, improve care quality, and promote a culture of excellence within the team.

Requirements

To be successful in this role, you will need:

Registered Nurse qualifications and current registration with the NMC.Experience of working in community nursing or a related field.Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.Strong clinical skills, with the ability to assess patient needs and develop effective care plans.A commitment to ongoing learning and professional development, with a desire to stay up-to-date with the latest research and best practices in community nursing.

Location

This role is based in the Tweeddale locality, with the expectation that you will work in West Linton and Innerleithen in accordance with service demands. You will be required to travel to different locations within the locality, and may be required to work flexible hours, including evenings and weekends.
