Job Description

As a Senior Mental Health Practitioner with the Leeds and York Partnership NHS Foundation Trust, you will play a vital role in shaping the Crisis Resolution Intensive Support Service (CRISS) for working-age adults across Leeds. The successful candidate will have a strong background in mental health, experience in crisis assessments, and excellent communication skills.

The ideal candidate will be able to work within a community setting, operating as part of a multi-disciplinary team, and contributing to early discharge through home-based treatment. You will also be expected to provide leadership, management, and supervision to junior staff, including coordinating and supervising teams in the delivery and evaluation of care.

We are looking for a skilled and dedicated individual who can lead our West CRISS service towards excellence. The successful Occupational Therapist or Mental Health Nurse/Social Worker will be responsible for undertaking crisis assessments, implementing home-based treatment interventions, and collaborating with inpatient and community services to facilitate seamless transitions for service users.

Key Responsibilities:

  • Lead and manage a team of healthcare professionals in providing high-quality care to patients in their homes.
  • Conduct comprehensive crisis assessments, develop care plans, and implement interventions to support service users.
  • Collaborate with inpatient and community services to ensure effective transition planning and continuity of care.
  • Provide clinical and managerial supervision, appraisal, and staff development to junior staff members.
  • Maintain accurate records, adhere to professional codes of practice, and report incidents as required.

Requirements:

  • Possession of relevant qualifications (e.g., occupational therapy or mental health nursing).
  • At least two years of post-registration experience in mental health settings.
  • Ability to drive and possess a valid driver's license.
  • Flexibility to work shifts, including evenings, nights, and weekends.
