Patient Care Coordinator Role

We are seeking an experienced Patient Care Coordinator to join our team in the Borders Specialist Dementia Unit.

In this role, you will be responsible for developing and implementing personalized care plans that promote independence, dignity, and well-being for individuals living with dementia.

The ideal candidate will have a minimum of SVQ Level 2 in health or social care and be passionate about delivering person-centered care.

You will work collaboratively with our multidisciplinary team to ensure that each patient receives high-quality care and support.

Key Responsibilities:

  • Develop and implement personalized care plans for patients
  • Work collaboratively with the multidisciplinary team to ensure high-quality care and support
  • Contribute to maintaining a safe and therapeutic environment
  • Communicate effectively with patients, families, and healthcare professionals

Salary and Benefits

We offer a competitive salary range of £24,907 - £30,451 per annum, depending on experience. You will also receive ongoing training and development opportunities and a supportive and inclusive work environment.
