Support Worker - Care Leavers

This dynamic role presents an opportunity to make a profound impact on the lives of young people transitioning into adulthood. As a Personal Advisor, you will play a vital part in helping them navigate this critical phase of life.

Your primary responsibility will be to develop and maintain positive relationships with your assigned young people, ensuring their unique needs are addressed and goals are achieved. You will collaborate closely with these individuals to create and implement personalized Pathway Plans, which will serve as a roadmap for their growth and development.

Other key duties include:

  • Creating and updating comprehensive case records
  • Scheduling regular home and community visits
  • Attending meetings and collaborating with partner agencies
  • Providing practical, personal, and emotional support

We are seeking an empathetic and skilled professional who can demonstrate effective communication and interpersonal skills. A strong ability to work in line with corporate parenting principles is essential, as is a passion for empowering young people to achieve their full potential.

Salary: £16.26 per hour (37 hours per week)
