About the Role

We are seeking a highly skilled and experienced Diabetic Nurse to join our multidisciplinary team at Ringmead Medical Practice. The ideal candidate will have a strong background in general nursing, with specialized skills and knowledge in diabetes care.

Key Responsibilities
  • Patient Care:
  • Provide comprehensive nursing care to patients with acute and chronic health conditions, including diabetes management and education.
  • Conduct thorough assessments of patients with diabetes, including monitoring blood glucose levels, HbA1c, and other relevant health indicators.
  • Develop, implement, and evaluate individualized care plans for patients with diabetes, ensuring optimal outcomes and quality of life.
  • Offer education and support to patients and their families about diabetes management, including diet, exercise, medication, and lifestyle modifications.
  • Perform routine health checks and manage any complications related to diabetes, ensuring timely interventions and effective care.
  • Clinical Duties:
  • Review diabetic patients and optimize their care plans, ensuring evidence-based practices and best outcomes.
  • Initiate and titrate insulin as needed, working closely with other healthcare providers to ensure comprehensive care.
  • Collaborate with other healthcare providers to coordinate comprehensive care for patients with complex needs, ensuring seamless transitions and effective communication.
  • Participate in chronic disease management clinics and group education sessions, promoting patient engagement and empowerment.
  • Patient Education:
  • Provide personalized education and counseling to patients on diabetes self-management, empowering them to take control of their health.
  • Develop and distribute educational materials and resources to enhance patient understanding and compliance, ensuring effective communication and engagement.
  • Conduct group education sessions and support groups for patients with diabetes, fostering a sense of community and shared understanding.
  • Quality Improvement:
  • Participate in practice audits, quality improvement initiatives, and research projects related to diabetes care, ensuring continuous improvement and excellence.
  • Stay up-to-date with the latest developments in diabetes management, integrating evidence-based practices into patient care and ensuring optimal outcomes.
  • Maintain accurate and thorough documentation of all patient interactions and treatments, ensuring transparency and accountability.
  • Professional Development:
  • Engage in continuous professional development and training related to diabetes care, ensuring expertise and excellence in practice.
  • Attend relevant workshops, conferences, and seminars to enhance knowledge and skills, staying current with best practices and innovations.
  • Mentor and support other nursing staff and students within the practice, promoting knowledge sharing and professional growth.
Requirements
  • Registered Nurse (RN) qualification with current NMC registration.
  • Specialized training or certification in diabetes care, diabetes education, or diabetes management.
  • Strong clinical skills and ability to manage complex cases, ensuring effective care and optimal outcomes.
  • Excellent communication and interpersonal skills, ensuring effective collaboration and patient engagement.
  • Ability to work independently and as part of a multidisciplinary team, ensuring seamless care and effective communication.
  • Proficient in the use of electronic health records and other healthcare technology, ensuring efficient and effective care.
