Job Summary

We are seeking a dedicated and experienced Diabetic Nurse in Primary Care to join our multidisciplinary team. The ideal candidate will have a strong background in general nursing, with specialized skills and knowledge in diabetes care.

Main Responsibilities

  • Patient Care
  • Provide comprehensive nursing care to patients with acute and chronic health conditions.
  • Conduct thorough assessments of patients with diabetes, including monitoring blood glucose levels, HbA1c, and other relevant health indicators.
  • Develop, implement, and evaluate individualized care plans for patients with diabetes.
  • Offer education and support to patients and their families about diabetes management, including diet, exercise, medication, and lifestyle modifications.
  • Perform routine health checks and manage any complications related to diabetes.
  • Clinical Duties
  • Review diabetic patients and optimize their care plans.
  • Initiate and titrate insulin as needed.
  • Collaborate with other healthcare providers to coordinate comprehensive care for patients with complex needs.
  • Participate in chronic disease management clinics and group education sessions.
  • Patient Education
  • Provide personalized education and counseling to patients on diabetes self-management.
  • Develop and distribute educational materials and resources to enhance patient understanding and compliance.
  • Conduct group education sessions and support groups for patients with diabetes.
  • Quality Improvement
  • Participate in practice audits and quality improvement initiatives related to diabetes care.
  • Stay up-to-date with the latest developments in diabetes management and integrate evidence-based practices into patient care.
  • Maintain accurate and thorough documentation of all patient interactions and treatments.
  • Professional Development
  • Engage in continuous professional development and training related to diabetes care.
  • Attend relevant workshops, conferences, and seminars to enhance knowledge and skills.
  • Mentor and support other nursing staff and students within the practice.

Requirements

  • Registered Nurse (RN) qualification with current NMC registration.
  • Specialized training or certification in diabetes care.
  • Strong clinical skills and ability to manage complex cases.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in the use of electronic health records and other healthcare technology.
