Job Summary: We are seeking a highly skilled and experienced Senior Strategic Planning Director to lead our Partnership's Strategy and Transformation activity.

Key Responsibilities:

  • Ensure financial responsibility and accountability is everyone's role;
  • Support a whole-system approach, centred around people in our communities.

As Head of Strategic Planning and Transformation, you will be responsible for:

  1. Primary care
  2. Health improvement
  3. Strategic planning
  4. Policy and performance
  5. Equalities and Human Rights
  6. Our approach to funding and grants
  7. Technology enabled care
  8. Moving forward our ambitious transformation programme.
