Salary

The estimated salary for this role is £46,148 - £52,809 per annum, pro rata.

Job Description

This role involves improving clinical outcomes for patients by enabling them to function independently, increasing their choice to remain in their own home/community, and reducing the need for or preventing inappropriate hospital admission.

You will work collaboratively with local GP and Integrated Teams in Swadlincote, providing support to patients in their own homes and in the Oakland bedded unit which supports patients discharged from hospital.

Required Skills and Qualifications
  • RGN or HCPC accredited degree
  • Evidence of post-registration clinical professional development
  • V300 Independent Non-Medical Prescriber
  • Clinical Decision Making course at Level 7 or equivalent
  • Teaching qualification
