About the Role

We are seeking a highly skilled Mental Health Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our Primary Care team, you will play a vital role in delivering high-quality mental health care to our patients.

Job Responsibilities
  • Contribute to the development and implementation of the Primary Care Mental Health Service model, working closely with colleagues to ensure seamless transitions between teams and services.
  • Work closely with Primary Care teams, including social prescribers and recovery navigators, to support adults with mental health needs, promoting early assessment and treatment.
  • Facilitate mental health and strengths-based assessments in Primary Care, providing targeted learning opportunities for staff to develop their knowledge and capabilities.
  • Build and maintain strong relationships with Primary Care Networks, Community Mental Health Teams, Social Care teams, and voluntary sector organizations to achieve seamless transitions and support for people with mental health challenges.
  • Promote understanding and confidence in the delivery of mental health care, providing credible clinical expertise and facilitating the development of safe and effective mental health care practice within the primary care setting.
Requirements
  • Professional qualification in mental health, equivalent to Registered RMN, Registered Social worker, or Registered Therapist.
  • Current registration with a professional body and commitment to Continuing Professional Development (CPD).
  • Significant experience in assessing risk and developing risk management strategies.
  • Strong track record of diplomacy and ability to maintain strong relationships, even in challenging situations.
