Key Responsibilities- Facilitate seamless discharge from hospital to home, ensuring a smooth transition for patients and their families.
- Conduct thorough home risk assessments to ensure the suitability of the home environment for clinical procedures.
- Administer intravenous medications, including parenteral nutrition and outpatient antibiotic therapies, to patients in their own homes.
- Empower patients and their carers to achieve full independence in managing their own care, promoting self-sufficiency and confidence.
- Advocate for patients' needs and interests, ensuring their voices are heard and respected.
- Registered General Nurse with current NMC qualifications.
- Excellent communication and interpersonal skills, with the ability to work effectively as a lone practitioner.
- Confident in managing workload and prioritizing tasks, with strong time management skills.
- Ability to escalate clinical or emergency situations, remaining calm and composed under pressure.
- Flexibility to travel and adapt to changing circumstances.
- Community or TPN experience an advantage, but not essential as comprehensive training will be provided.
